Most melee champs have an ability for damage mitigation and the most important thing about playing melee in a team fight is knowing when to use that skill. That ties into the "knowing the damage" thing. If you can maximize your damage mitigation, you can stay in a fight longer and have a much better chance of getting kills.
I've been thinking that ganking basically comes down to knowing how much damage you can do with the window of opportunity you have. It just seems like it's a very rare thing for more advanced players to put themselves into positions where they can be easily killed, specially in lane.
