The other accident was when the space shuttle Challenger exploded during lift off when a leak through the right solid rocket motor aft joint caused "some" problems and lead to the liquid oxygen tank being broken up and causing a big explosion and stuff. This happened like Jan. 28th 1986?
The Columbia had cracked heat titles from lift off and they hoped there wouldn't be any troubles on reentry but as you can see there were.
It sucks.
