Honestly, it's your champion selection. You usually pick champions with skill shots or other location-based effects that you only have so much control over. A Cho'gath that can't hit his AoEs is useless. Doesn't matter if it's you or HotshotGG.
Lux and Teemo are two other champions that come to mind pretty readily. Missing a cage on Lux is always painful. Teemo is... well... ok: personal opinions about the champion aside, if your team isn't working to bait enemies into Teemo shrooms or the enemies just aren't hitting them, then he's really bringing very little to the table.
I would say switch over to someone like Rumble/Kennen/Kassadin/Cass/Karthus/etc who does have a skill shot, but they're on low cooldowns and you're not "punished" as hard for missing them. I know you dislike the "no-skill" style of targeted nukes, but I think you let that influence your champion choice a little too much.
As for farming... I don't have anything to say about it, really. I disagree with Meer, though, largely because, no matter which champion you're playing, you often try to fill that "support" role of giving farm to the rest of the team. At least that's been, like, 90% of the games I've played with you.
EDIT: Another thing that may or may not happen (can't always tell) is getting flustered in team fights or "high stress" situations. More than once it's seemed to me like shit has happened and then you kind of process what went down after the fact. If that is the case -- and not just a misinterpretation on my part -- then I suggest getting into more Dominion games. It kind of forces you to deal with that cluster-fuck scenario.
