First and foremost, are custom characters allowed?
No. You can 'customise' existing named characters from DBZ to an extent, like getting a scar and changing their personality, but basically no.
Is there a pre-made template for character submission or can I use my own?

Is there a list anywhere of who plays which character on the site?
Your username is the character you play. The roster is the currently played characters.
How many characters may a user register, and at what time? (Meaning, if we can have more then one, is there a waiting period between each registration?)
Only one character. If you're found to be playing more than one, you'll be banned permanently (staff can check IPs).
Are there any untaken characters from the series that are off limits? (I did not observe anyone registering any of the Supreme Kais, which surprised me somewhat)
Kais, Dragons and Hildegarn are all off-limits. Anyone on the inactive list with (NPC) next to their name is also off-limits.
I noticed that in the inactive character page, there were numerical stats, while in the bio pages, there were not. I'm wondering why this is.
Bio pages are for a description of your character. There's no point putting your numerical stats there because that would mean staff would have to update everyone's tech vault with new stats every sunday. Stats just stay on the roster.
And finally, are character deaths and kills determined by a GM? Stats? Or the players themselves?
Deaths have to be authorised by a member of staff. In a fight, players will roleplay back and forth until they've agreed to finish, then a member of staff will judge the 'winner' and decide who dies. Your success is based on your roleplaying ability, but obviously stats are a deciding factor too.

You get TP every week which you put into stats. You can put points into int, which doesn't increase your powerlevel but for every 50 points nets you a new 'Best Move'. You start off with 3, one for each 50 int. So not 6
